Clinical Studies on Pituitary Adenoma

Abstract
Pitnitary adenomas comprise 9 to 18 percent , of all intracranial tumors. By far the great majorty are chromophobe adenomas, which enlarge the sella as they grow. In addition to producing the signs and symptoms of hormone excess, these tumors may compress and destroy normal pituitary tissue ¢¥ within sella turcica and produce hormonal defi ciency states, or they may extend out the sella turcica to compress nervous structure in the vicinity.
Fifty-one cases of pitutary adenoma have; been analyzed. on clinical manifestations and laboratory studies. These include patients in whom the diagnosis of pituitary adenoma has been made over the last 11 years from January, 1965 till Dec ember, 1975 at Severance Hospipital.
Tho results were as follows:
1. Among 51 cases, chromophbe adenomas were 38 cases, acidophilic ones were 12 cases, and,
mixed, form was 1 case.
2. The 51 cases with pituitary adenoma studied were 18 to 64 years old. Among chromophobe
adenoma, more than 50% of patients were in the second and third decades, but patierts with
acidophilic :adenoma were=diffusely distributed & from first., decade to sixth decade and one
patient with mixed from was in the second decade.
3. The male and female ratio of pituitary adeiioma was 1.7 1, that of chromophobe adenoma was 2.211, and that of acidophilic adenoma was
4. Chief complaints of patients-with chromophobe adenoma- were, visual symptom (92.1%), head
ache 165.8%), endocrine symptom (15.8%) and vomiting in decreasing order, but those with acidophilic adenoma -were visual symptom (41.7%), headache (41.7%), endocrine symptom (41.7%) and general weakness..
5. In 35 of 38 patients with chromophobe adenoma, and in all patients with acidophilic adenoma, sk,pll films demonstrated sella enlargement.
6. The diagnostic accuracy of brain scan, pneumoencephalogram, :carotid angiogram, EEG and CSF exam were compared. The positive resul is of the carotid angiogram was 88. 2%, and that of the pneumoencephalogram was 88.3%.
7. The results of thyroid function test and B. M. R., and the amount of urinary 17-ketosteroid and urinary 17-hydroxycorticosteroid were variable, These results were not diagnstic in differentiating the types of pituitary adenoma.
